Vehicle Damage (both City owned and third party)

In the drop down box the information provided shows the options for the amount of damage or loss to vehicles and equipment.  Select the points for the appropriate amount which is categorized by:

  • None   (0 points)

  • Minor Damage (scrapes or small dents) (+1 point)

  • Moderate Damage (broken replaceable items (mirrors, bumpers, ect.) Large dents, deep scratches, deployed airbags)  (+2 points)

  • Severe Damage (Vehicle(s) must be towed away. Collisions likely to pose a significant threat to your health such as head trauma, broken bones, whiplash, ect) (+3 points)

  • Major Damage Total loss of vehicle (+4 points)

Loss of Property

Property damage is categorized by the actual total value of repairs and/or replacement of all property involved (both City owned and third party).

In the drop down box the information provided shows the options of points for total cost of repairs and/or replacement cost of property excluding vehicles/equipment.  Select the points for the appropriate total value of property damage that is categorized by:

  • None  (0 points)

  • Minor Damage (Mostly cosmetic damage, ie. dented mailbox, minor denting of downspouts, ect  (+1 point)

  • Moderate Damage (Repairable non-structural damage ie. Mailbox knocked over completely, damaged siding)  (+2 points)

  • Severe Damage (Structual damageor other significant damage that requires extensive repairs)  (+3 points)

  • Destroyed (Total Loss) (+4 points)

 

Environmental Factors

In the drop down box the options displayed for these environmental conditions are categorized by:

  • Main Cause - operating in severe weather, severe ice storm, tornado warning, severe snow storm, severe thunderstorm, etc.  (-2 points)

  • Factors Present - operating in non-severe rain, snow, sleet, fog, etc. (-1 point)

  • None - weather was not a contributing cause for the incident (0 points)


Severe weather will be determined by the National Weather Service.

Contributing Factors

The contributing factors may include, but are not limited to, speeding, inattentiveness, fatigue, moving violations (driving wrong way on a one way, driving with a suspended/revoked license, failed/improper lane/run off road, fail to obey traffic controls, fail to signal intentions, fail yield right of way, following improperly, improper backing, improper lane change, improper passing, improper turn, improper pushing/towing), inattentive, interfered by passenger, overcorrecting, reckless/negligent driving, exceeding the speed limit, swerved or avoided, vision obstructed, aggressive driving/road rage, careless/erratic driving, violation of City policy, texting while driving, distracted driving, cell phone use, negligence, etc.

Choose the number of factors that have contributed to the incident. There may be more than one. Each contributing factor is one (1) point. Combine all points for a total not to
exceed 4 points. NOTE: Utilize the police report to assist with determining total contributing
factors for the at fault MVA.

Points are cumulative and all factors must be taken into account.

Prior On-The-Job Moving Violations

How many moving violations has the employee obtained while conducting City business in the past 4 years?

In the drop down box the options displayed for the number of moving violations are categorized by selecting:

  • 1 moving violation  (+1 point)

  • 2 moving violations  (+2 points)

  • 3 or more moving violations  (+5 points)

Was the incident reported in a timely manner without delay?

In this final section the form inquires if the employee immediately reported the incident to their supervisor.  The criteria for “without delay” is defined as, “notification shall be made by the employee as soon as it is safely possible.”  Furthermore, mitigating circumstances will be considered in this section.  A delay for not reporting immediately must be justified due to unknown circumstances that may arise during an incident.  

These types of justifications for not reporting immediately are, but not limited to, the following: the employee is severely injured, unable to notify because of mass loss of communications (cell towers damaged or not available, radio system down, land lines un-operational etc.) or road conditions/severe weather inhibited the capability to make immediately notification.

Below are the listed quantitative sums that will assist the Department Head in determining the proper corrective action.  The listed actions are only recommendations and the final decision will only be made after the employee is afforded the appropriate due process.

  • Below 0        Remedial training

  • 1-2 points        Verbal reprimand

  • 3-4 points        Written reprimand

  • 5-6 points        1 day suspension

  • 7-9 points        2 or more days suspension

  • 10 or more points - Up to termination, to be determined by the Department Head
